TLDR : Answer Summary
The required form to extend a 'visa exempt on arrival' is the Application for Extension of Temporary Stay in The Kingdom (TM.7), which can be downloaded from the Immigration Bangkok website. However, it is essential to note that the application must still be submitted in person, as a photo is taken upon application.
Well here in Phuket all Needs about 10 Minuten only

I will be happy if it only takes 90 minutes. What counter do you have to visit to get a queue ticket? Can I skip the first long queue that you see when you first enter the building?
that should do it. Just did mine there yesterday. Only one photo. 30 people in que ahead of me. About 90 minutes total.

Its an application for an extension of stay... as Robert has stated...for 30 days...And must be applied for in person as your photo is taken upon application.
