Do the forms 

Chaengwattana Personal Information Form 1/2 // Foreign national information (file section)

need to be filled out or is it just the the tm7 form ?

i didn´t found that while searching here.

The personal information form was scrapped a good while back. Just the TM.7 form and copies of your passports data page, your arrival stamp and your TM.6 departure card. Those things a passport sized photo and 1900baht get you a 30 day extension
