The extension is easily approved. I've never heard of a denial. Get the 90 day insurance.

Can a USA citizen obtain a visa for 90 days in Thailand for travel on December 31, 2020?

Not true the Special Tourist Visa is 90 days and can be extended/renewed twice for a total of 9 months.

If you are trying to stay long term you do NOT want the Single Entry Tourist Visa (SETV) you want ths Special Tourist Visa (STV).

STV is 3 months and can extend to 9 months.

Is proof of onward travel required for a Single Entry Tourist Visa in Thailand?

You DO need proof of onward travel when applying for a SETV. But I there is a trick almost everyone does.... Book on Expedia (24 hour free/no fee cancelation. Book your tickets. Print out the required info. Cancel ticket. Everyone who does border bounces does this.
