We will be applying for a 30-day visa extension to our 45-day visa exemptions received in mid-November at Bangkok international airport. We are Canadians and will apply in person at the Central Festival Immigration office in Chiang Mai. Question #1: For the very first time, we did NOT fill in a TM.6 arrival/departure card when arriving from Taipei on EVA Air. Is this going to be a problem when we apply for a visa extension? We have only the small date stamps for 45 visa exemption in our passports. Question #2: I have also heard that some tourists here are being asked for TM30 forms from their hotels. Any comment on that? Thanks.

TLDR : Answer Summary
The TM6 arrival card has been temporarily discontinued for air travelers, so not filling it out should not be a problem when applying for your visa extension. However, you will need a TM30 form from your hotel, as it is required for the extension process in Chiang Mai. If your hotel did not provide the TM30 upon check-in, you may need to request a hard copy directly from the TM30 office, which can lead to longer wait times. Overall, ensure to gather all necessary documents before visiting the immigration office.
In CM, the Hotel Screen Shot of TM30 is not acceptable. They will have OP go to back of building to the TM30 Office to "convert" to hard-copy of TM30 Slip. No biggie, though there can be a big line, so OP should either go early, or perhaps get the TM30 Slip on a different day and return for Extension. (I would Posted this Addendum, but you turned off comments).
ohh my... you know as well as I do that the TM6 has been temporarily discontinued for those who enter Thailand by AIR.
AND we are receiving reports of it still being Issued to those who enter Thailand by LAND BORDER crossing.
Does that pass muster with you today?

The TM 6 has been temporarily discontinued for those who Fly into Thailand.
The TM 30 is filed by your Hotel on check-in... If they did not ask for your passport on check-in. .ask them for a screenshot of their filing of the TM30.

The tm6 has been cancelled for now., for air traveller’s only. No worries about that. In Chiang Mai, you’ll need the tm30. Bangkok immigration doesn’t ask for it.
