What do I need to know to renew my retirement extension and get a reentry stamp in Jomtien?

We are going to Jomtien tomorrow to renew our retirement extensions. We also want to get a reentry stamp. Is there a form for that, or anything else we need besides 1000 baht?

I just looked at the form online and it appears we need another photo for that application. Will we also need the usual copies from our passports?
