Im in Thailand on a 60 day tourist visa already extended for 30 days. On my return to the UK I what to change to a A/O multi entry visa.
Question is using the medical form printed from Thai Embassy London web site, can I get it filled in / endorsed in a Hospital in Udon Thani and be acceptable for my AO Multi Visa application

The reason I say they might accept a thai signed medical form IS
I had someone use one getting an O-A visa from the thai consulate in Los Angeles without a problem BUT another person who used the thai consulate in Chicago who tried to use one signed by a thai doctor had to get one from the US instead.
