If my STV last extension finishes 7th December, and I go to my local immigration office in Trat on 26th November for a Covid extension, will it start the day I'm in there or when stv finishes?

Each immigration office has their own policy. Talking to Rayong won't do any good if you live in Trat. If they say no, then show proof of rebooked airline tickets showing you are planning to leave the country as soon as possible.
I recently had my flights cancelled on the 30 th November..so been trying that one..understand the 11 day part which I may have to forfeit..but there will be more flights early new year..and I'm midway with my vax program which will make it a bit easier also..

That's assuming you even manage to get the COVID extension in the first place. That's no guarantee for someone on an STV.
