I have metv need to be out by Sept 23 want to extend it in Chiang Mai for the 1900baht what are the requirements for the extension because my flight is booked after the 23rd because the cost was 250$ cheaper please any help would be awesome as I want to go do it this week

Form is called TM 7, you can down load from the files section of this page and print, it is one page printed front and back side. Or you get one at the Immigration Office.
No, they won't photocopy it for you, you have to get the photocopies and passport photos before you show up to the immigration office.
