So, my Volunteer visa will expire November 2.  I am told there is a chance they will not renew/extend.  Kinda makes sense.

What is require for US citizen to get Covid extension?  Will I require a letter from my embassy?

You will need to cancel your volunteer visa. I'm assuming the visa was issued from the agent. If so, you will have to contact the agent to cancel the visa and have them issue the COVID-19 extension.

Generally, they have not been requiring anything in the way of proof. There have been a few scattered incidents of people not getting approved but generally you should just be able to apply for the covid extension without any supporting documentation.

The problem is that he used an agent. If he used an agent, the immigration office (PROBABLY) will not cancel his existing extension of stay so he can go on the 60 day extension of stay based on unable to travel due to COVID-19

Honestly not sure which agent I used. I used the Agent that shares a parking lot, with the immigration office in Jomtien.

1 year + 3 months, 60,000. But that was one year ago. I asked them if they could do it again and they are not able. It seems nobody is offering Volunteer visas at this time.

Look in your passport. Is the first line on your "extension of stay" stamp says Khon Kaen or another immigration office that's NOT Pattaya or in Chonburi?
