How can I successfully apply for a DTV visa based on dental treatment in Thailand?

I am in a similar situation. I had some dental work done for a crown and need some additional work (future implant and crown). I got the work done at a government dental office and got a signed and officially stamped letter from them saying I should get an implant done on my next visit, however there are so specific dates or payments. I am currently in Vietnam and wondering if I should apply online, seems like a

shot I have at getting the DTV visa at best. Maybe a gamble worth the $300. What do you think?
